A new video created by the Healthcare Intelligence Network (HIN) demonstrates the benefits of the medical home approach for individuals with diabetes -- vastly improved clinical outcomes and patient compliance at a reduced cost. Manasquan, NJ (PRWEB) April 3, 2008 -- When two healthcare organizations applied the framework of the patient-centered medical home model to the care of diabetics in their populations, they experienced dramatic improvements in patient compliance, outcomes and costs. Nearly 75 percent of healthcare organizations responding to the non-scientific survey said they were aware of the medical home model, up from 50 percent in a 2006 survey. HIN has summarized its latest survey findings in a complimentary white paper, Medical Home Awareness and Early Successes, which also details how healthcare organizations are building medical homes from the ground up.
