Orion Health Customers Recognized for Innovations in Chronic Disease Management at the Calgary International Chronic Disease Management Conference

Calgary Health Region and Capital Health of Alberta, Canada, and Procare of Auckland, New Zealand have were awarded Best IT Projects in Chronic Disease Management at the Calgary International Chronic Disease Management Conference. Using Orion Health Chronic Disease Management solutions, the organizations have developed innovative approaches to managing chronic diseases for better patient health outcomes.

Calgary Health Region and Capital Health of Alberta jointly won first prize for the Best IT Project in Chronic Disease Management in Canada. The regional chronic disease management project, initiated in 2002, features an innovative digital dashboard project unifying disease management across the entire province. With more than 40,000 patients under care and including home care and family physician involvement, the system assists patients and physicians with the management of diabetes, hypertension, COPD, pain, anticoagulation management and community care.

Procare, Auckland, New Zealand, received the international award for the best Chronic Disease Management Program - the conference's highest accolade in this category. In 1997 Procare recognized a rapid and unsustainable growth in admissions and ambulatory response. By implementing a chronic disease management program the organization was able to dramatically reduce the growth in acute medical admissions from 9 percent to 0 percent.

Also presenting at the conference was Orion Health customer Alere Medical, who demonstrated their unique approach to using technology to improve care and reduce costs for patients with chronic diseases including heart disease, Coronary Artery Disease (CAD), Chronic Obstructive Pulmonary Disease (COPD), asthma and diabetes.
